Price for services:
$95 new patient first visit (effective June 1st, 2011)
$75 basic sick visit (add $20 for Federal Holidays, 11.25, 12.24 & 12.31)
$50 subsequent sick sibling established patient (same visit)
$85 camp/back to school physical (includes full history, physical with ht/wt/BP/vision/hearing, etc)
Additional Charges/Services
$20 extended visit (over 30 minutes)
$10 strep test
$10 each nebulizer treatment
$5 pulse oximetry
$50 LiquiBand (and related supplies for laceration repair)
$10 earrings
$10 prescription only
$10 school or camp forms
Medications charged separately
A copy of your visit with charges will be provided for you for each patient seen to submit to your insurance (or flexible spending account) if desired or required, and to follow up with your primary care provider. Payment is expected at time of service via cash or check.
